APPLYING TO INTERNSHIP

1. The application form with field of study,

addressed to Rector of JSC «KAZMUCE»

2. Original of medical degree diploma

3. The copy of medical degree diploma

4. Original of medical degree diploma transcript

5. The copy of medical degree diploma transcript

6. 3 copies of national ID

7. 6 photos (size 3 x 4 centimeters)

8. 086у medical certificate (e-form) + chest x-ray image

9. The copy of military identity card.

10. Certificate of educational grant ( if available)

11. Receipt of payment (for tuition on a fee basis)

12. For foreign citizens: a copies of passport, - personal identification number registration certificate, - visa for the Republic of Kazakhstan

13. Folder and 10 files

14. A 4 size envelope and 1 envelope for photos

15. The list of scientific works and other documents confirming Applicants’ personal achievements (if available)

16. Academic reference or Transcript (for persons who have completed higher professional education abroad )

17. Document nostrified in the Republic of Kazakhstan about completion of the previous level of education abroad
